Financial Aid Application
Deadline and Required Documents
Thank you for your interest in Mounds Park Academy for the 2012-13 academic year. You may begin to apply for financial aid starting on January 13, 2012 as described below. To be eligible for priority consideration for financial aid at Mounds Park Academy, you must complete your online application and submit all required tax documents electronically to School and Student Services (SSS) no later than Thursday, March 1, 2012. We will communicate decisions about financial aid on Thursday, April 5, 2012.
We use School and Student Services (SSS) to process financial aid applications. Based on the financial information you provide in the Parents’ Financial Statement, SSS gives us an estimated amount your family can contribute to educational expenses. This estimate is a starting point to help us make fair and objective financial aid decisions. To begin your financial aid application for the 2011-12 school year, please follow these steps below. (Please note: You only have to complete one Parents’ Financial Statement, even if you have several children applying.)
COMPLETING YOUR ONLINE PARENTS’ FINANCIAL STATEMENT (PFS)
- Starting on January 13, 2012 go to sss.nais.org/parents.
- Click on the “Apply Now” button.
- You will set up a PFS Online account using your e-mail address and a password.
Returning parents: If you completed an online PFS application last year, use your e-mail address and existing password. If you have forgotten your password, click on “Forgot my password.”
- If you are separated, divorced, have never been married or share custody of your child(ren), MPA requires that the non-custodial parent complete a separate PFS application form and include financial information as well. (Note: The person who signs the MPA contract will be considered the custodial parent).
- Submit and pay for the PFS ($35). This fee is nonrefundable.
ADDITIONAL DOCUMENTS YOU MUST SUBMIT ONLINE WITH YOUR PARENTS’ FINANCIAL STATEMENT (PFS)
Our school requires that you submit to SSS the following documents in addition to your completed Parents’ Financial Statement:
- A signed copy of your 2011 Federal Income Tax Return IRS form 1040 (or equivalent) if you are the parent(s), stepparent or guardian(s). Be sure to include copies of all supporting Miscellaneous Schedules filed with your tax return.
- Copies of all W-2 or 1099 forms for 2011 from parent(s), stepparent or guardian(s).
- If you are self-employed or own a business, you must submit copies of your Schedule SE or E and your Schedule C form – or any S Corporation additional forms you file along with your 2011 tax return that are related to your business.
HOW TO SUBMIT YOUR REQUIRED ADDITIONAL DOCUMENTS ONLINE WITH SSS
Please be advised that your application for financial aid WILL NOT be considered until all required documents have been received. It is the responsibility of each family to manage their file and ensure accuracy and completeness.
- Go to the Manage Documents tab in the PFS Online. There you can upload documents from your computer.
- Save each document separately in Adobe Acrobat .pdf or as a .jpg file. Each document cannot exceed a size of 500 kilobytes.
- On the Manage Documents page, click on “Browse,” locate the document on your computer, then click on “Upload.”
** MPA’S NEW POLICY FOR THE 2012-13 FINANCIAL AID PROCESS **
- Your application for financial aid will not be considered until all required additional documents have been received by SSS. If you are not sure if your file is complete, check your Manage Documents tab within your PFS Online account to confirm.
