Our goal at Mounds Park Academy is to ensure that each student we accept experiences success. As the teacher in the required subject of mathematics, you have been asked to complete this form in support of a student's application for admission. The MPA Admission Committee would be grateful if you would provide the information as requested below. Your assistance is essential in evaluating the applicant and, therefore, a completely candid and thorough evaluation will best serve the interests of the applicant, the applicant's family, and the school. This information will be kept confidential.
