Based on the information provided, a realistic appraisal of school resources, and feedback from the family and student, MPA may create a support plan which provides accommodations to help meet the needs of the student. This support plan is shared with the student’s teachers, and our best effort will be made to address the student’s individual needs. Plans are formally evaluated annually, and updated as needed with feedback from parents, teachers, administration and the student. To develop this plan, we need access to the following: Any academic testing and related recommendations The accommodations provided at the student’s last school, if applicable Any IEP or 504 Plans that have been in place.
